Clover, Boston
The fast food chain, which operates popular food trucks and restaurants in Cambridge and Boston, was closed in the summer of 2013 after being linked to a 12-person Salmonella outbreak. The inspector found spoiled cauliflower as well as hummus and salads coming back from food trucks at improper temperatures. Clover reopened a week later.
